A Brazilian scientist may revolutionize the diagnosis of Alzheimer’s disease with a simple saliva test! Gustavo Santos, from Unicamp, developed at the Ribeirão Preto Medical School of the University of São Paulo (USP) a method that can identify the disease up to 20 years before the first symptoms appear, by analyzing biomarkers such as the pTAU protein. The test is non-invasive, inexpensive, and easy to apply on a large scale, which could greatly facilitate early detection — essential for slowing the progression of the disease through lifestyle changes. With an estimated 75 million cases of dementia worldwide by 2030, the study represents a ray of hope — and is now seeking investors to turn the test into reality.

Gunderson Dettmer advised Replicate Bioscience, Inc. in a collaboration agreement with Instituto Butantan, a leading nonprofit producer of immunobiologic products in Brazil. Replicate is a clinical-stage company amplifying the power of RNA therapeutics by pioneering its novel self-replicating RNA (srRNA) technology with potential improvements in bioactivity, induction of more potent immune responses, and improved therapeutic index. Under the agreement, Instituto Butantan will obtain exclusive rights to develop and commercialize Replicate’s novel srRNA rabies vaccine RBI-4000 in Brazil and other Latin American countries. A historic milestone for Intrials and for the future of gene therapy in Brazil. On this November 24th, during the meeting of the Executive Group of the Economic-Industrial Complex of Health (GECEIS) of the Brazilian Federal Government, a very important announcement was made: ANVISA’s authorization for GEMMABio’s phase 1/2 clinical trial for SMA type 1, a decisive step for patients, families, and the future of advanced medicine in Brazil. Intrials is directly involved in this project, contributing its clinical research expertise to support GEMMABio and Fiocruz in advancing this innovative clinical program. We are proud to be part of a collaborative effort that strengthens the national health ecosystem. I just completed the Biotech Unveiled course by RA Capital Management and highly recommend it to anyone interested in how the drug development process intersects with the US healthcare system. Growing up in Brazil, I was fortunate to be among the 25% with private insurance. Our universal healthcare system, while fantastic, isn’t without flaws - but I rarely worried about access or affordability. That changed when I moved to the US nearly nine years ago. From out-of-network providers to co-pays and specialty drugs, I had to learn a completely new – and often frustrating – system. High premiums that don’t guarantee coverage and a lack of transparency highlight the absurdity of health insurance in this country. Here are a few insights from the course that stuck with me: 🔹 Drugmakers’ true customers are not sick patients – it’s the mostly healthy people who pay more into insurance than they take out. This is where healthcare dollars originate. 🔹 Branded drugs account for only 8% of total healthcare spending. And unlike everything else, they decrease in cost over time. Granted, not all medicines can or will go generic but there are policy fixes that could address this ‘breach’ in the biotech social contract. 🔹 Current Cost-Effectiveness Analysis methodologies often miss the full value of innovation. By incorporating caregiver impact, productivity gains, and broader societal benefits, the industry can do a much better job at demonstrating why we cannot give up on funding valuable innovation. A temporary and limited solution (can I even call it that?) with many unintended consequences. Lowering/eliminating out-of-pocket costs is a far more powerful way to make medicines affordable without stifling innovation. Every day, I help biotech companies bring transformative medicines to market. But beyond advancing our industry, I want to be a stronger advocate for policies that sustain innovation and ensure that its benefits are accessible to all.

ANVISA’s Next Regulatory Agenda: What’s coming for 2026–2027! The Brazilian National Health Surveillance Agency (ANVISA) has built a strong reputation as one of the world’s leading regulatory authorities since its creation in 1999 (yes, the year before the Y2K scare, remember that?! Easily-scared kid Heverton does!). Back in 2009, ANVISA introduced its Regulatory Agenda (Agenda Regulatória – AR) to plan, prioritize, and increase transparency in its regulatory actions, with the ultimate goal of helping stakeholders anticipate and prepare for upcoming changes— essentially a win-win for both regulators and industry. Last month, the Agency wrapped up its public consultation to gather input for the 2026–2027 Regulatory Agenda, as part of ANVISA’s 2024–2027 Strategic Plan. The preliminary list is now live: 115 proposed regulatory themes across 16 macro areas —from cosmetics to pharmaceuticals, medical devices and more! As someone passionate about medical devices from product development, quality, and regulatory perspectives, I am selfishly crossing my fingers for themes 92–94 to make the final list. Topics like the regulation of IVDs, reprocessing of medical devices, and updates to SaMD regulations are hot discussions worldwide. Strengthening the safety and effectiveness of medical devices benefits everyone, and it would be fantastic to see one of the world’s top agencies prioritizing these themes in the years to come. 🔬 Redefining vaccination: Instituto Butantan and Valneva break the mold #Brazil has approved the first vaccine based solely on #immunogenicity - a breakthrough that redefines #regulatory frameworks and opens new opportunities in public health and emerging #markets. On April 14, Brazil’s health regulatory agency officially approved the chikungunya vaccine, developed by Butantan and Franco-Austrian pharmaceutical company Valneva. The true innovation? Instead of comparing infection rates between vaccinated and #placebo groups, approval was based on lab data showing 98.9% of participants developed strong #neutralizing antibodies (The Lancet Group).
