Because adhoore concepts confidently likhe jaate hain, & confidence gets mistaken for correctness. As an Ayurveda researcher & consultant, it's not my job to argue with sources — my responsibility is to create awareness, not confusion. Now, if you actually understand Ayurveda, the "selection process" isn't confusing at all — it's just logic: 1. 𝐅𝐫𝐮𝐢𝐭 𝐚𝐧𝐝 𝐦𝐢𝐥𝐤 𝐜𝐚𝐧'𝐭 𝐛𝐞 𝐦𝐢𝐱𝐞𝐝 Just an incomplete rule. It's sour & unripe fruit that clashes with milk — the acid curdles the protein. Sweet, ripe fruit? Totally fine. So avoid sour/raw fruit + milk, not fruit + milk as a category. 2. 𝐎𝐚𝐭𝐬 𝐚𝐫𝐞 𝐧𝐨𝐭 𝐥𝐨𝐜𝐚𝐥 𝐟𝐨𝐨𝐝. 𝐋𝐞𝐭𝐬 𝐦𝐞 𝐦𝐚𝐤𝐞 𝐃𝐚𝐥𝐢𝐲𝐚 Not friction — this one's actually right. Daliya, cooked soft with ghee, is warm, local, and easy on digestion. Ayurveda's whole thing is eating what your body (and your geography) recognises. 3. 𝐑𝐨𝐥𝐥𝐞𝐝 𝐨𝐚𝐭𝐬 𝐨𝐫 𝐬𝐭��𝐞𝐥-𝐜𝐮𝐭? Doesn't need to be a crisis. Steel-cut retains more of its natural structure & nutrients but needs longer, gentler cooking to become digestible; rolled oats are more processed and quicker. Either works — just cook it properly. 4. 𝐈 𝐰𝐚𝐧𝐭 𝐟𝐫𝐞𝐬𝐡 𝐟𝐨𝐨𝐝, 𝐧𝐨𝐭 𝐫𝐞𝐟𝐫𝐢𝐠𝐞𝐫𝐚𝐭𝐞𝐝 This isn't the dumbest take. Fresh, warm food = more prana. Refrigerated, reheated food = more ama (residue your body has to work harder to clear). Science may disagree; Ayurveda doesn't. 5. 𝐈 𝐝𝐨𝐧'𝐭 𝐥𝐢𝐤𝐞 𝐜𝐨𝐥𝐝 𝐟𝐨𝐨𝐝, 𝐈 𝐰𝐚𝐧𝐭 𝐬𝐨𝐦𝐞𝐭𝐡𝐢𝐧𝐠 𝐡𝐨𝐭 Also correct, not fussy. Warm food kindles digestive power. Cold food dampens it. Your gut isn't being dramatic — it's following the manual. 6. 𝐍𝐨 𝐩𝐫𝐨𝐭𝐞𝐢𝐧 𝐩𝐨𝐰𝐝𝐞𝐫 — 𝐜𝐚𝐧 𝐈 𝐮𝐬𝐞 𝐫𝐚𝐢𝐬𝐢𝐧𝐬 𝐚𝐧𝐝 𝐡𝐨𝐧𝐞𝐲 𝐢𝐧𝐬𝐭𝐞𝐚𝐝? Ayurveda would lean toward raisins, dates, or honey over a processed powder — these are considered whole, sources of sweetness with additional nutrients (iron, fibre), not "empty calories." Just don't cook the honey — add it after, off the heat. 7. 𝐈 𝐦𝐢𝐠𝐡𝐭 𝐟𝐨𝐫𝐠𝐞𝐭 𝐭𝐨 𝐜𝐚𝐫𝐫𝐲 𝐢𝐭 𝐭𝐨 𝐨𝐟𝐟𝐢𝐜𝐞 Fair — set a reminder. Not every problem needs a Sanskrit explanation. CHECK FIRST CAPTION FOR 8th, 9th and 10th point. Did you notice — I didn't have to override any of these. I just had to UNDERSTAND them. That's what real coaching looks like. Taste is information. 4️⃣ Sip warm water, never cold — cold water douses your digestive fire (agni). 5️⃣ Chew till it's almost liquid — digestion starts in the mouth, not the stomach. 6️⃣ Eat your biggest meal at lunch — agni peaks with the sun, not at 9pm. 7️⃣ Walk 100 steps after eating — Ayurveda's og post-meal ritual (Shatapavali). 8️⃣ No screens, no stress talk at the table — your body can't digest food and fear at the same time. Aur sabse dangerous baat ye hai ki hum ise "bas thoda stress hai" bol ke ignore kar dete hain. 5 signs jo aap shayad roz feel karte ho par notice nahi karte: 1️⃣Chhoti chhoti cheezein bhi bahut bhaari lagti hain. Kaam ho toh jaata hai, par ek simple email bhejne mein bhi jaise poori taaqat lag jaati hai. 2️⃣ Busy toh hain, par kaam ka kaam nahi ho raha. Poora din to-do list mein ulझe rehte ho, par jo actually important hai wo baar baar taal dete ho. 3️⃣ Dimaag mein dhund si chha jaati hai. Sochne ki clarity gayab, aur relax karne ki koshish karo toh bhi dimaag mein vichaar chalte hi rehte hain. 4️⃣ Feelings jaise numb ho gayi hon. Jo cheezein pehle khushi deti thi, ab unse koi farak nahi padta — na khushi, na dukh, bas ek flatness. 5️⃣ Neend poori lene ke baad bhi thakan nahi jaati. She was a mid-level manager at a corporate firm, caught in the middle of constant office politics. Every day felt like a battlefield — unnecessary comparisons, backbiting, and the pressure to "prove herself" again and again. Slowly, it started showing up in her body: ✓ Sleepless nights ✓ Racing thoughts, and ✓ Anxiety that followed her even into weekends. When we started working together, one thing was clear — the workplace wasn't going to change overnight. But her response to it could. We worked on: → Recognising negative thought loops before they spiralled → Building small, non-negotiable wind-down routines to protect her sleep → Separating "what happened at work" from "what it means about her" → Simple breathing and grounding techniques for high-stress moments Within a few weeks, the politics around her hadn't disappeared — but her reaction to it had changed. Better sleep. Calmer mind. And for the first time in a while, she felt in control again. 𝗣𝗼𝘀𝘁 𝟭 𝗼𝗳 𝟰 — Pathogenic & Likely Pathogenic: How Strong Evidence Is Built 🔬 𝗪𝗵𝗮𝘁 𝗧𝗵𝗲𝘀𝗲 𝗖𝗹𝗮𝘀𝘀𝗶𝗳𝗶𝗰𝗮𝘁𝗶𝗼𝗻𝘀 𝗠𝗲𝗮𝗻 ACMG/AMP 2015 (Richards et al.) is a qualitative evidence-combination framework. Probability thresholds were formalised by Tavtigian et al. (2018): • Pathogenic → >99% posterior probability • Likely Pathogenic → >90–99% posterior probability Not a label. A probability conclusion. 🧬 𝗣𝗩𝗦𝟭 — 𝗧𝗵𝗲 𝗦𝘁𝗿𝗼𝗻𝗴𝗲𝘀𝘁 𝗖𝗿𝗶𝘁𝗲𝗿𝗶𝗼𝗻, 𝗕𝘂𝘁 𝗡𝗼𝘁 𝗔𝘂𝘁𝗼𝗺𝗮𝘁𝗶𝗰 Applies to null variants — nonsense, frameshift, canonical splice ±1/2, initiation codon, exon deletion — in genes where LOF is the confirmed disease mechanism. • What experienced analysts know that pipelines miss: Not every LoF-looking variant qualifies for PVS1. • Abou Tayoun et al. (2018) moved the field from "variant type = PVS1" toward "predicted consequence + gene/disease mechanism = PVS1 strength." Apply PVS1 blindly to gain-of-function disorders — misclassification follows. ⚠️ 𝗦𝗽𝗹𝗶𝗰𝗲 𝗩𝗮𝗿𝗶𝗮𝗻𝘁𝘀 𝗔𝗿𝗲 𝗡𝗼𝘁 𝗔𝘂𝘁𝗼𝗺𝗮𝘁𝗶𝗰𝗮𝗹𝗹𝘆 𝗣𝗩𝗦𝟭 • The same ±1/2 splice variant may cause exon skipping, in-frame deletion, intron retention, or be rescued by alternative splicing — each yielding different PVS1 strength levels. • ClinGen SVI (Walker et al., 2023, PMID: 37352859) provides a framework for incorporating RNA experimental evidence into PVS1 strength using PVS1_Strength (RNA). • Computational splice prediction contributes as PP3 — supporting evidence only. • It does not justify PVS1. 🔥 𝗧𝗵𝗲 𝗜𝗻𝗱𝗲𝗽𝗲𝗻𝗱𝗲𝗻𝗰𝗲 𝗣𝗿𝗶𝗻𝗰𝗶𝗽𝗹𝗲 • Evidence codes should not be stacked. • Computational splice prediction (PP3), RNA confirmation, and the LoF consequence describe the same biological event at different resolution levels — not three independent streams. • This distinction separates a pipeline output from a defensible clinical classification. 🔑 𝗞𝗲𝘆 𝗧𝗮𝗸𝗲𝗮𝘄𝗮𝘆 • Clinical sophistication means applying not just ACMG/AMP 2015 — but ClinGen VCEP gene-specific refinements built precisely because the original framework was intentionally broad. • PVS1 is the most powerful criterion in the framework. • Used correctly — definitive. • Used automatically — dangerous. He was a dedicated researcher, an inspiring academician, and a person with a positive mindset and willpower. Having lost his mother in 1947, just 48 hrs after Bharat’s independence, and his father in 1956, he changed the course of his career to suit the challenging financial conditions, though he wanted to be a CA as he was a good student of Commerce. Nevertheless, he became the first person in #Bihar to be awarded a #PhD in the Faculty of #Education. He was also the first and only person that year to receive a #UGC fellowship to pursue a PhD in Bihar, in the Faculty of Education, for which he received ₹150 per month. His life journey continues to inspire and motivate me.

As a nutritional medicine practitioner, I find this shift fascinating and, honestly, a little concerning. Yes, protein matters!! It builds and repairs tissue, supports enzyme and hormone function, and keeps us satiated. For years, many diets genuinely fell short on quality protein, especially among older adults and those recovering from illness. So the renewed focus isn't misplaced — it's overdue in many ways. But here's what I see in my practice: people swapping whole, nutrient-dense meals for protein bars and shakes, chasing gram counts on an app instead of listening to their bodies. A few things worth remembering: - More isn't always better. Beyond a certain threshold (roughly 1.2–2.0g/kg body weight for most active adults, individualized to kidney health, age, and goals), extra protein doesn't build extra muscle — it just becomes extra metabolic work. - Source matters as much as quantity. A protein bar loaded with sugar alcohols and isolates is not nutritionally equivalent to lentils, nuts, eggs, fish, or paneer, which a have fiber, micronutrients, and satiety built in. - Protein doesn't work alone. Muscle synthesis depends on adequate nutrition, resistance training, sleep, and overall caloric sufficiency. Obsessing over protein while neglecting these is like tuning one instrument and ignoring the orchestra. - Digestive load is real. I regularly see patients with bloating, fatigue, or kidney-function concerns tied to excessive protein supplementation, particularly when hydration and fiber intake haven't kept pace. Build your plate around whole foods first — dals, eggs, dairy, fish, poultry, legumes, nuts. Use supplements to fill genuine gaps, not as a lifestyle identity. Trends are useful when they nudge us toward better habits, and harmful when they replace nuance with numbers. Food is information for the body — not just fuel, and definitely not just macros!!Vd. My team has now completed 150. In April 2021, I finished my robotic surgery training in Goa. The hospital had sent me there and backed me before I had done a single case. She left us on 2 June 2021. The first robotic case I did came at the end of July, just under two months after losing her. Something she told me before that has stayed with me. Since the management had sent me for training, the right thing was to justify their faith by putting it into practice. Here is where five years have taken us. 📍 150 robotic gynaecological surgeries completed 📍 Ectopic pregnancy, endometriosis, fibroids and ovarian cysts 📍 Hysterectomy, fertility enhancing surgery, sterilisation reversal 📍 Cervical and endometrial cancer surgery None of that is a solo number. Nurses, technicians, anaesthetists, support clinical departments and administration have supported us over the years and made every one of those cases possible. It is the team's milestone before it is mine, and it belongs most of all to my patients who trusted us with something new. Bhavna Sodhi Manchanda, Anya and Anvi have put up with every mood along the way, and none of this happens without them. My mother was way ahead of her time. She kept pushing me to learn and try new things, always for better outcomes and better care for my patients. Once again, thank you to my family, my team, and my patients for keeping the faith in me. Here's to the next milestones.

EDGENOMICS
In India, we urgently need an honest conversation about who should practice genomics—and whether the current DM Medical Genetics training actually prepares clinicians for it. Based on the official competency documents, here are some uncomfortable truths: The program is still fundamentally “medical genetics,” not modern genomics. The curriculum is heavily rooted in classical genetics, dysmorphology, pedigree analysis, and syndrome-based diagnosis. Genomics—large-scale sequencing, data interpretation, and computational pipelines—is mentioned, but not built as the central clinical skillset. Bioinformatics and variant interpretation are listed as competencies—but not as core training depth. The guidelines expect familiarity with databases, software, and variant interpretation tools, yet there is no structured, intensive training in algorithmic analysis, pipelines, or large-scale genomic data handling. This creates a dangerous gap between what is claimed and what is actually taught -sometimes putting patients at risk. The model assumes clinicians will interpret complex genomic data without deep computational grounding. The curriculum expects DM graduates to interpret NGS and microarray results and reassess variants using databases and software. But without rigorous training in bioinformatics, statistics, and sequencing technology, this becomes superficial interpretation rather than true genomic expertise. Training is structured like an organ-based clinical super-specialty—not a data-driven genomic discipline. The documents explicitly state that medical genetics training follows the same structure as other clinical super-specialties, centered on history, examination, differential diagnosis, and treatment. Genomics, however, is fundamentally a computational, population-scale, and data-intensive science. The program focuses heavily on counseling and phenotyping rather than high-throughput genomic analysis. A large portion of competencies revolve around counseling, dysmorphology, pedigree drawing, and communication skills. While essential, these are not substitutes for deep expertise in sequencing technologies, variant curation frameworks, or genomic analytics. There is no clear requirement for hands-on experience with real-world genomic pipelines. The uncomfortable conclusion: A DM in Medical Genetics, as currently structured, may produce competent clinical geneticists—but not necessarily competent genomic physicians. Genomics is no longer just syndrome recognition plus a test order. It is a data science discipline embedded in medicine. If training does not include deep exposure to sequencing technologies, bioinformatics, variant science, and population-scale datasets, we risk creating clinicians who order genomic tests without truly understanding the data they are interpreting. And in a field where results are heritable, irreversible, and socially consequential, superficial expertise is not just inadequate—it is dangerous.
