As of 2026, 25 states plus D.C. have some level of mandated fertility insurance coverage, yet the laws vary widely and there are limitations to eligibility criteria that leave many still uncovered. Employer-sponsored fertility benefits fill these coverage gaps and provide allocated funds for treatment.
