Angela has built and scaled provider networks at Aledade, TailorCare, Sword and more. She brings a wealth of knowledge and network connections to help us scale. She is known for being a connector, a closer, and a true believer in what value-based care can do. 🔵 Kristie Koch — VP of Care Operations & Innovation. Kristie is a Six Sigma Black Belt and VBC execution expert who turns big strategy into real, measurable results. Most recently, she served as Chief Performance Officer at Matter Health, where she operationalized a high-performing value-based care program for elderly populations. We’re proud to see the continued expansion of our partnership with Humana, alongside Novocardia, Karoo Health, and US Heart and Vascular. At Chamber, our focus is on supporting cardiologists with the infrastructure and care coordination needed to deliver high-quality, value-based cardiovascular care. As this collaboration grows, more Medicare Advantage members will have access to proactive, evidence-based support designed to improve outcomes and reduce avoidable hospitalizations. We’re committed to building a more connected, outcomes-driven future for heart care, together.
We're building on our existing cardiac care partnership with Novocardia by teaming up with Karoo Health, US Heart and Vascular, and Chamber to expand access to coordinated, value-based care for eligible Medicare Advantage members. Together, we're offering around-the-clock clinical guidance, remote monitoring that helps identify issues earlier, and integrated care planning between cardiologists and care managers, with support that now reaches members across more than a dozen states. This expansion reflects our commitment to making care easier to navigate and ensuring our members have timely, proactive support throughout their heart-health journey. As we continue to evolve and bring new capabilities forward, these partnerships strengthen how we support healthy aging and enhance the overall member experience for the people we serve. The Frist Cressey Ventures team is thrilled to share that we have partnered with Chamber to bring value-based care to cardiology, unlocking better outcomes and meaningful cost savings in one of the most expensive specialties in healthcare. Bill Frist, M.D., one of FCV's founders, began his career as a cardiothoracic transplant surgeon who saw—up close—how system gaps impact patient lives. That perspective guides our conviction in Chamber and its work to enable earlier identification of risk, more coordinated care, and better cardiovascular performance visibility for providers and payers. Proud to lead Chamber’s $60M Series A and to continue to transform healthcare and improve lives with co-founders George M. Aloth and Sameer Sheth, M.D. and AlleyCorp, General Catalyst, American Family Ventures, Company Ventures, Healthworx, Black Opal Ventures, and HSBC Innovation Banking.
