This Breast Cancer Awareness Month, Flatiron Health is marking a milestone: our Breast Cancer Panoramic dataset has surpassed one million patients, with real-world data spanning the full breast cancer continuum, from early-stage through metastatic disease. The dataset includes 97,000+ patients tracked from early to metastatic disease, 575,000+ HR+ patients, and 109,000+ HER2-positive patients, with ongoing HER2 ultra-low integration. That depth supports biomarker-defined cohort research, treatment sequencing analyses, and real-world comparator arm development, turning complexity into clarity for the subgroups that matter most. Over 48,000 people are diagnosed with colorectal cancer in the UK every year. Yet, fragmented and largely unstructured data has long slowed down the research that could improve care and save lives. By working collaboratively with NHS partners, Flatiron Health UK is helping to change that. We're building a national dataset that reflects how care is actually delivered – updated every 3 months, with depth of variables needed to power research and improve patient care. At Leeds Teaching Hospitals NHS Trust, this approach is already succeeding by accelerating how quickly teams evaluate trial eligibility. By joining this effort, NHS organisations get access to highly curated patient data that power research and care, global benchmarking data from Flatiron’s to identify improvement opportunities, and income to reinvest in cancer services. Two times more patients. Three new geographies. One consistent data model. Flatiron Health has expanded its global Panoramic datasets with AI-powered breast cancer and NSCLC real-world data across the UK, Germany, and Japan—the second multinational Panoramic launch, following January's prostate cancer debut. Built on the same common data model underpinning Flatiron's US breast cancer and NSCLC Panoramic datasets—spanning nearly 1.5M patients—these new datasets unlock 2x the patients previously available across both tumor types and geographies, validated against Flatiron's VALID framework and built by clinical teams local to each market. For HEOR, RWE, and Medical Affairs teams, that means consistent cohorts and comparable outcomes across markets—complexity turned into clarity, ahead of ESMO 2026. When ESR1 mutation testing wasn't routinely performed by the NHS, there was no UK patient data to build an external control arm for elacestrant's NICE submission. The evidence needed to satisfy the UK's PICO framework simply didn't exist locally. Menarini Stemline leveraged high-fidelity, US-based, multimodal #RWD from Flatiron Health and Foundation Medicine. With rigorous cross-border adjustments to account for population differences, the analysis demonstrated a clear outcome advantage for elacestrant: 7.4 vs. 4.6 months median PFS and 27.7 vs. 18.0 months in overall survival. In February 2025, NICE recommended elacestrant for NHS coverage. Locally-sourced data is often preferable — but it isn't always available. This case shows that research-ready #RWE, built to the right standard of quality and completeness, can fill critical evidence gaps and support HTA submissions across borders. Flatiron’s VALID framework has laid the foundation for a more rigorous, transparent way to measure the quality of LLM-extracted data—not just at a single point in time, but continuously as models, data and clinical practice evolve. By establishing a consistent approach to evaluating whether AI-generated data are fit for purpose, VALID helps turn data quality into something that can be systematically measured, monitored and improved. A new editorial in JCO Clinical Cancer Informatics from Danielle Bitterman, Umit Topaloglu and Jeremy Warner reinforces the framework’s impact, pointing to VALID as a model for the continuous validation the oncology informatics field should adopt as a standard for LLM-based evidence generation. For years, oncologists have debated whether tumor mutational burden (TMB) can reliably predict who benefits from immunotherapy. A new study, co-authored with Foundation Medicine using our joint Clinico-Genomic Database, just brought some of the clearest evidence yet—and it's earned the Journal for ImmunoTherapy of Cancer's (JITC) 2026 Outstanding Paper Award for Immunotherapy Biomarkers. The study looked at more than 8,000 patients across 24 cancer types—the largest real-world analysis of its kind. The takeaway: patients with higher TMB consistently lived longer on immunotherapy alone, reinforcing the FDA-approved threshold used to guide treatment decisions today. It's a meaningful step toward answering a question that's shaped immunotherapy practice for years. Congratulations to the team on this well-earned recognition.
