Several years ago, Epic and Cerner (now Oracle Health) revolutionized healthcare operations software in US hospitals. Epic was known for offering a structured configuration approach, while Cerner yielded to endless customization demands to cater to customer needs. Today, the focus has shifted from demanding software adaptation ("EHRs should adapt to the way I practice!!) to valuing EHR vendors' configuration expertise, with each snowflake melting into the laminar flow of optimized workflows. The era of individualized software solutions is giving way to the practice-based evidence of proven EHR implementations. Who better than Judy Faulkner herself, the visionary behind Epic, to address the communication differences between the Northeast and the Midwest. She recently hypothesized that "Midwest nice" might be causal to a communication gap that exists between her staff and salty Northeast executives who are might be hard on the outside, but soft and swishy on the inside. Oracle (Cerner) falls even further behind Epic, and its new AI-powered EHR is only going to make things worse for them. Yesterday, a new KLAS report dropped an expected bombshell: Oracle Health lost 57 hospitals in 3 years as clients fled to other platforms (mostly to Epic). Meanwhile, Epic has surged ahead, now serving 42% of U.S. acute-care hospitals (nearly double Oracle’s 23%). Oracle is betting on a new AI-powered EHR (whatever that means) to regain ground. But here’s the reality: healthcare isn’t a bleeding-edge industry when it comes to tech adoption. Doctors and patients are cautious. At Callen-Lorde, when we rolled out Abridge’s AI scribe, we had some providers opt out, not because it didn’t work, but because they weren’t comfortable with AI in their exam rooms. Some patients and staff also voiced concerns. In an LGBTQ+ community where many have experienced trauma and are also highly informed, that skepticism is totally understandable even if I don’t fully agree with it. This isn’t just Oracle’s challenge, it’s every healthtech innovator’s. The question isn’t “Can AI do it?” but “Will patients and providers trust it enough to use it?” This Epic-versus-Oracle saga is also a litmus test for digital health innovation and trust. "Hours Without Training": Decoding Oracle's Bold EHR Promise Oracle claims their new EHR deploys "within hours without training." Having architected platforms for thousands of healthcare organizations, here's what this likely means. Day 1 Reality What Works: Patient lookup, simple notes, recent labs What Doesn't: Complex orders, specialty templates, billing, prior auth, referrals "No training" = they've simplified it so much, only basics work. Wait until a cardiologist needs procedure documentation or pediatricians need growth charts. The Coexistence Truth Running new Oracle alongside Cerner sounds smart. Based on similar implementations, expect: -Dual infrastructure costs -Workflow disruption (switching systems) -Data synchronization challenges -Which system is source of truth? AI Translation Risks Their "semantic AI layer" bridging old/new systems? Healthcare is full of ambiguity: -Abbreviations with multiple meanings -Date format confusion -Dosage interpretation variations -Even small error rates compound at scale Real Timeline -Ambulatory now: 20% complexity -Acute 2026: 60% complexity -Full replacement: Last 20% takes 80% of time Why? Zero tolerance for ICU/OR downtime. What Oracle Gets Right Despite challenges, this is revolutionary: -No more 1990s technical debt -True cloud elasticity -API-first innovation -Voice-first matches clinical reality Bottom Line Implementation will be messier than promised. But Oracle has patience and capital to lose money for 5+ years perfecting this. For those who've spent careers duct-taping healthcare systems, Oracle's willingness to start fresh is refreshing. The question isn't if they can build better - it's whether organizations can survive the transition. Experience suggests: organizations that succeed treat this as transformation, not just implementation. One of the hardest parts of healthcare IT leadership is making decisions when there is no perfect answer. In mission-critical, nonprofit healthcare environments, leaders are often asked to choose between options that all carry risk—financial, operational, clinical, or reputational. Waiting for certainty isn’t always an option, and over-engineering can be just as dangerous as under-planning. Over time, I’ve learned that effective risk management isn’t about eliminating risk altogether. It’s about: -Understanding where risk truly lives -Being clear about what the organization can and cannot tolerate -Making decisions that are defensible, transparent, and aligned with the mission -Preparing teams and systems to respond when—not if—something goes wrong Strong IT and cybersecurity leadership shows up long before an incident ever occurs. It’s present in governance structures, vendor relationships, training, documentation, and the quiet discipline of maintaining systems that simply work. In healthcare, the goal isn’t perfection. It’s resilience.
