Dear FMOS Management / Senior Management, Subject: URGENT ESCALATION – SERIOUS FMOS PROCEDURAL AND REGULATORY FAILURE FMOS Reference Nos.: FMOS17145 and FMOS17146 I wish to formally and urgently escalate my serious concerns regarding the handling and closure of my complaints under the above FMOS reference numbers. These matters have already been ongoing for approximately six months, during which I attempted to resolve the issues through the Financial Service Provider (FSP) and Bank Negara Malaysia (BNM). After those avenues failed to adequately address the substantive issues, and based on legal advice, I escalated the matter to FMOS. A police report was also lodged in relation to the matters raised. The police themselves asked whether an investigation was required, as the report appeared to contain elements potentially involving criminal conduct.

Reopen or otherwise conduct a proper supervisory review of both matters. 3. Confirm whether all documents couriered to FMOS were received and reviewed. 4. Identify the officer(s) responsible for the assessment and closure decision. 5. Provide the reasons and procedural basis for closing the complaints. 6. Confirm whether the supporting evidence was reviewed before closure. 7. Provide a written explanation of the applicable complaint-handling workflow and status notification process. 8. Ensure that my complaints are reviewed by a senior officer who has not previously made the closure decision. 9. Preserve all records, correspondence, call logs, case notes and documents relating to FMOS17145 and FMOS17146. 10. Provide me with a formal written response addressing each of the above concerns.
