Final Carnage is a episode replacement PWAD for Doom II and the GZDoom source port, created by Onox and initially released on July 6, 2023. It contains 15 maps as of the latest build (13 regular and two secret), alongside many new graphics and sounds. It is set in a large UAC facility overtaken by the forces of Hell, set across many levels as the player explores its different floors and the outskirts of the buildings. It is heavily inspired by Half-Life and Duke Nukem 3D, featuring realistic locales, destroyable objects, and plenty of ambient sounds. The author recommends pairing the mod with Project Brutality, but it also fully works on its own.
Final Carnage was given an honorable mention at the 2023 Cacowards.
The v0.2 release on May 21, 2025, added six new levels, new custom content, and many improvements and fixes.[1]

Besides various props, the mod also includes a new type of enemy called gore nest (thing type 6531): it has 500 hit points and is generally tied to some arena fights in the later levels, making killing it necessary to continue.
