High/Low 5 is a single-level PWAD for Doom II and a limit removing source port that was released in July 2013. It was designed by Christian Hansen (c-cooper) as the fifth and final entry in his High/Low mini-series, and uses the music track "Death's Bells" by L.A. Sieben, from TNT: Evilution. It was recognized as one of the runners-up at the 20th Annual Cacowards.

- Head south from the starting lift, through the wall that lowers, then onto another lift. Ride it back up and head south to get a backpack. (sector 1333)
- In the middle of the map, take the door west of the bridge leading to a chaingun, then around to a switch. Take the lift up, head north, then west to another switch. Use that one, then head back to the starting point. Go back south, and past the wall that has lowered. Just to the right is a teleporter taking you above a megaarmor on a crate. (sector 1290)
- The same switch mentioned in secret #2 lowers some bars a bit south of the crate with the megaarmor. Go there for the wall ahead to lower. Jump there, open the door, and press on the damaged wall to the east. Fall down to get a soul sphere. (sector 346)
- Go to the steps that trigger the lift leading to the red door, then jump east and into an open crate to get a rocket launcher. (sector 1297)
- In the northeast hall, flip the switch and run past the crusher to the open door before it closes. The bars ahead of you will lower. Drop into the bigger room here, then enter the trench where the switch is and use it. Quickly head northeast to the hall that lowers and cross over to the other side with a medikit. Use the switch here to get a chainsaw. (sector 1027)
- Ride the lowered hall back up and go south and southwest. In this room, use the switch, then run southwest into the next room and go towards the two barrels. You will hear a door open. Quickly run back to the previous room before it closes and into the small room to the southeast before it closes too. Use the lift and head south to pick up a megasphere. (sector 590)
- In the building surrounded by the moat (the blue key building), get to the southwest side and shoot the switch to the south. This raises wooden pillars allowing you to head west all the way to a teleporter. When you take it, press the switch to your left. This will lower a teleporter to the south of the bridge with two doors. Get to the west side of the bridge to access the teleporter, which will take you to a blur artifact. (sector 298)
- In the southwest cave, take the westernmost lift up, then go to the southwest end and jump to a rock ledge. Enter the cave to find a box of shotgun shells, an energy cell pack, and a medikit. (sector 1378)
