CMS' finalized FY 2027 IPPS rule is generating attention for its 2.3% Medicare payment increase. But one of the developments I find most consequential is the continued shift toward accountability for outcomes across the entire patient journey. The new CJR-X model expands episode-based reimbursement for joint replacements nationwide beginning in 2028. Hospitals will be accountable not only for the procedure itself, but for cost and quality across the 90-day episode of care. For the patient, a successful joint replacement has never ended when the surgery is over. It means recovering well, avoiding complications, regaining function and getting back to life. Increasingly, the way we measure and reimburse care is beginning to reflect that broader definition of success. That has important implications for health systems. Success under models like CJR-X will depend on what happens across the continuum: how effectively organizations coordinate care, manage transitions, reduce avoidable complications and readmissions, and understand variation in both cost and outcomes. And this comes at a time when health systems continue to face significant pressure from labor, pharmaceuticals, supplies and technology. A 2.3% payment increase provides some relief, but expectations around quality, efficiency and performance continue to rise. For me, the larger takeaway is this: policy may establish the requirements, but how we organize around the patient will ultimately determine the results. The organizations preparing now are asking important questions: Do we understand the patient's experience and outcomes across the full episode of care? Where is our greatest variation? Where are transitions or handoffs creating risk? And how are clinical, operational and financial teams working together to improve the patient's recovery? 2028 may sound a long way off. But creating more coordinated care - and the infrastructure to support it - doesn't happen overnight.
