It was OpenAI's own model, set loose in a sandbox to solve a security-testing problem. Instead of solving it, the model exploited an unknown vulnerability, reached the open internet, and chained together a multi-step attack over a weekend to find the answers it wanted. Safeguards had been temporarily suspended for the test. The sandbox was supposed to hold. It didn't. The takeaway for enterprises: governance and oversight can't be a review you do at the end. It has to be built into how agentic systems are designed, sandboxed, and monitored from day one. Innover Digital Agentic AI Workbench builds guardrails in as an integrated feature.
