Ira Kirschenbaum MD shared thisThis is a slide from a talk I recently gave to a room of corporate healthcare executives called "On Leadership & Management in Healthcare." Leadership in healthcare and orthopaedics specifically has to come from within the specialty. Not from administrators or outside consultants. Leaders first INQUIRE: They need to know everything about the terrain and climate of the environment they are in. They most importantly need to know deeply about the Attending Surgeons. Leaders do better by managing by behavior vs managing by objectives. Leaders then REQUIRE: They require, by mutual agreement, that everyone in the department simply does their job (a Bill Belichek reference). They require of themselves to stay true to the mission of the department. Leaders finally INSPIRE: The people working in the department do best when they are inspired to do the best surgery, the best research, and the best overall patient care. Surgeons are NOT commodities, they are THOROUGHBREDS! Pure and simple. It is an honor and privilege for a patient to agree to allow them to enter their bodies through an incision. That comes with the responsibility to assure you are at the top of your game for them. While they are thoroughbreds, they do have to run on a racetrack. That, most often, should be determined by the Department Chair who is entrusted with executing the global mission of the department. My analogy is a baseball team. As my hospital is just 3 train stops from Yankee Stadium, I will use them for this. The hospital CEO is like Hal Steinbrenner, the owner of the team. He/She communicates direction and objectives of the health system. The Department Chair is Dave Cashman, the general manager of the team. He/She recruits surgeons, determines day-to-day operations, and hires the manager. The Aaron Boone manager (preferably a non-MD), sets the daily schedule, assures execution of the plans of the general manager, and makes sure everyone is in position to succeed. Aaron Judge, the player, is the Attending Surgeon. He/She must be in an environment set by those I just mentioned, free of distractions and irritations, so hits and home runs can flow. Run strong and hard!

Residency programs register on a first-come, first-served basis. Each program selects its three strongest research abstracts. Abstracts do not need to be ready when the program registers. The opening three rounds are held virtually. During each matchup, competing programs alternate presenting their research live. Independent judges evaluate each presentation. Scores from all three abstracts are combined to determine which residency program advances in the bracket. Winning benefits the entire program. At every stage of the competition, the advancing team earns prizes for its entire residency program—not only the residents who presented. The Final Four competes live. Ira Kirschenbaum MD shared thisI am just re-reading this amazing book. I used this slide at a talk I had the honor of giving at this year's CLINICAL ORTHOPAEDIC SOCIETY INC in Louisville, KY. To save you some time right now, immediately going out and reading this book: A Black Swan is an event that: 1. Seems highly unlikely or unpredictable beforehand. 2. Has an enormous impact. 3. Is explained afterward as though it should have been predictable. Classic Black Swans would be the Internet, the personal computer, and the COVID-19 epidemic. In Orthopaedics, to me, the development by Sir John Charnley of low-friction hip arthroplasty qualifies. What are some Black Swans in orthopaedics- historical ones and maybe even modern innovations developing now? Individual or industry developments?

Given this, an online support group exists for patients to gain support and advice from a community of patients with the same injury. Similar groups exist for other chronic medical conditions and traumatic injuries; such groups have been studied to determine the impact on the affected patients. There have been no prior studies specifically investigating the effect of these support groups on patients with tibial plafond fractures. The road to healthcare reform is paved with unintended consequences. What's not to like about price transparency? Patients shop for care, marketplaces arise, and providers compete on cost and quality. But that's not what happened. In reality, hospitals decided to just pay the penalties or publish indecipherable charge masters. Payers realized they could use the information as a negotiation tactic, pitting one group against another. If you're a small to mid-sized practice in a highly consolidated market, good luck competing. Futhermore, patients don't shop for care the way many (often outsiders) assume. Many still rely on brand, word of mouth, or doctor recommendations. Driven by churn and financial quarter-based incentives, buyers overindex on price. Quality and patient experience, equally important parts of the value equation, are secondary concerns. What seller wants to participate in a race-to-the-bottom marketplace where price transparency works against you? Like many things healthcare, the issue and its solutions are complex. Price transparency alone isn't enough when almost everything else is a black box and a willingness to play by the rules is used against you.
