That might have been tolerable when volumes were smaller and enforcement expectations were unclear. It is no longer defensible. Years of litigation, policy ambiguity, and fragmented enforcement have created a program where manufacturers are expected to fund discounts without consistent visibility or verification. That is not sustainable. In this Drug Channels Institute, an HMP Global Company article, Kalderos CEO Angie Franks lays out why waiting for regulatory certainty is the wrong bet. Mike Egan reposted thisMike Egan reposted thisEarlier today, a federal court in Maine blocked the government’s proposed 340B rebate pilot from going live on January 1, 2026. More details will emerge, and I'm sure there will be more perspectives as they do. But a few things are already clear. First, the growth of the 340B program remains rapid and unchecked, with HRSA reporting a 23% increase in spending on 2024 purchases. The rebate model is one way to address long-standing integrity challenges, but it is not the only solution. Second, while the 340B landscape remains unsettled and regulatory uncertainty is not going away anytime soon, what has become increasingly clear is that claims-level transparency is not only permissible but essential. Multiple federal courts have now affirmed that manufacturers may require full claims data as a condition of accessing the 340B price, with judges explicitly recognizing that transparency addresses fundamental gaps in the program. So, the question is no longer “Can claims be collected?” It is “Why wouldn’t they be?” This moment highlights a broader truth. The industry does not need more point solutions tied to a single policy experiment. It needs infrastructure built for complexity, change, and scale. That is why the work being done by Kalderos matters, and that is why #Truzo exists. Changing careers can be scary, especially when things around us seem so volatile, but rewards without risks are trivialities. Going into HR meant moving away from what was familiar and "safe," and studying for my exams meant a lot of time dedicated towards learning, studying, and practicing. However, now on the other side of it, I can happily say that those risks led to one of the most satisfying rewards to date in my professional life. Thank you to all who supported me, but I'm not stopping here - there's a lot more to come!
